2015 Newcastle Boys Representative Nomination Form

Please note:

U18 HNSW State Titles will be held 1-3 May 2015 at Narellan and the Moorebank carnival will take place on 26 April 2015. By nominating you are making yourself available to attend both carnivals.

The initial selection trial will take place on Friday, 13 March 2015 from 5.30pm to 7.00pm at the Hockey Centre. A fee of $10 per player for turf hire is payable on the day.

If you wish to nominate and will not be available to attend the selection trial please complete the nomination form and email kay.johnston@exemail.com.au advising of the reason that you are unable to attend.
