2015 Newcastle Boys Representative Nomination Form

Please note:

U15 HNSW State Titles will be held 22-24 May 2015 at Grafton and the Moorebank carnival will take place on 17 May 2015. By nominating you are making yourself available to attend both carnivals.

The initial selection trial will take place on Sunday 29/3/15 at 4.30pm to 6.00pm at the Hockey Centre. A fee of $10 per player for turf hire is payable on the day.

If you wish to nominate and will not be available to attend the selection trial please complete the nomination form and email kay.johnston@exemail.com.au advising of the reason that you are unable to attend.
